Guardian Authorization for Child Attending with Another Family Child 1(Required) First Last Child 2 First Last Child 3 First Last Parent/Guardian(Required) First Last Parent/Guardian Email(Required)Member Your Child(ren) Will Be Attending With(Required) First Last Consent(Required) I agree to the waiver below and give permission for my children to attend the pool with another family.By selecting “I Agree,” you acknowledge and agree that swimming and related activities involve inherent risks, including drowning, slips and falls, collisions, equipment use, and environmental conditions, and you voluntarily assume all risks associated with use of Ridgewood Pool Inc.’s facilities and participation in pool activities; to the fullest extent permitted by law, you release and waive any claims against Ridgewood Pool Inc. and its directors, officers, board members, employees, volunteers, contractors, and agents (collectively, the “Releasees”) arising out of or related to use of the facilities, including claims arising from the negligence of the Releasees, except for gross negligence or intentional misconduct; you are responsible for the conduct of any minor children included under your membership and any guests you bring to the pool and agree to indemnify and hold harmless the Releasees from claims arising out of their actions, including reasonable attorneys’ fees and defense costs, and you acknowledge that adult household members and guests must sign their own Ridgewood Pool waiver before using the facilities; you agree to comply, and to ensure your minor children and guests comply, with all posted rules and staff instructions, and access may be suspended or revoked for noncompliance; you further acknowledge that use of the facilities constitutes participation in a recreational activity under Wisconsin law and that nothing in this Agreement limits any rights, defenses, or immunities available to Ridgewood Pool Inc. under Wis. Stat. § 895.525 or other applicable law. Additionally, you grant Ridgewood Pool Inc. permission to photograph and video record individuals in outdoor spaces while at the facilities and to use such images or recordings for marketing purposes.
